EMR and ESD for Oesophageal Cancer

What is Endoscopic Mucosal Resection (EMR)?

Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are the main endoscopic techniques for the treatment of oesophageal cancer.

The right treatment will be dependent on the stage of your condition:

 

  1. Endoscopic Mucosal Resection (EMR) is used for the treatment of early-stage cancerous and precancerous growths.
  2. Endoscopic Submucosal Dissection (ESD) is used for the removal of large tumours.

Endoscopic mucosal resection (EMR)

Endoscopic mucosal resection works by focusing on a single target to capture the diseased tissue. It uses an electrosurgical current which cuts the tissue which has been targeted. An injection may be used to elevate the lesion but this is not always necessary and will depend on individual circumstances. It is usually used to remove lesions over 10mm.

Endoscopic submucosal dissection (ESD)

Endoscopic submucosal dissection is performed through highlighting the malignant lesion with a stain and then the surgeon injects a liquid into the submucosal layer beneath the lesion to lift it and make it easier to remove. The lesion can then be cut out and removed.

What is the Difference between EMR and ESD?

EMR is the easier and lower risk procedure of the two, so if it is an option, most doctors will recommend this first to their patients. It isn’t the best choice when larger lesions are found as it would take longer than ESD where the whole lesion can be targeted.

Research has shown that EMR also has a higher lesion recurrence than ESD, which means further therapy may be necessary.

Recovering after your Procedure

Both of these procedures require endoscope and so most patients experience some throat pain and soreness afterwards and a modified diet may be recommended to allow the incisions and scar tissue to heal. Your individual care plan will be described by your medical team after the procedure has been carried out.
